Here’s what we want you to know up front: many bleeding changes during perimenopause are a normal reflection of shifting hormone patterns. But “many” is not “all,” and not every bleeding change should be automatically chalked up to perimenopause without a second thought. What is happening?
Menstrual bleeding is regulated by the interplay of estrogen, progesterone, and ovulation. Estrogen stimulates the growth of the uterine lining (endometrium). After ovulation, progesterone stabilizes that lining and prepares it for a possible pregnancy. If pregnancy doesn’t occur, progesterone drops, and the lining sheds. That’s your period.
How ovulation changes affect your cycle
During perimenopause, ovulation becomes progressively less predictable. [1-2] Some cycles still involve ovulation with a relatively typical hormonal pattern. Others are anovulatory, meaning no egg is released, so the progesterone rise that normally stabilizes the lining doesn’t happen. [1]
Estrogen levels during this time don’t decline in a straight line either. They can fluctuate, sometimes climbing higher than usual before eventually trending down. [9] This combination of variable estrogen and inconsistent progesterone is what tends to produce the cycle-to-cycle unpredictability many women describe.
The STRAW+10 staging system, the gold standard for characterizing reproductive aging, defines the early menopausal transition as a persistent difference in consecutive menstrual cycle length of at least 7 days. [10-11] The late transition is marked by at least 60 days of amenorrhea (a skipped period) at least once. [2][10] In North America, the average duration of the menopausal transition is about 4 years. [1]
Why this happens during midlife
The underlying driver of bleeding changes in perimenopause is the natural decline in ovarian reserve, the gradual reduction in remaining egg supply that happens with age. [1] As this occurs, the signals between the brain and ovaries become less consistent, which affects both ovulation timing and the levels of estrogen and progesterone across a cycle. [1-2]
Common patterns during the transition
A few patterns are commonly described during this transition:
- Cycle length changes. Cycles may become shorter (more frequent periods) earlier in perimenopause, and then longer or more spaced out (skipped periods) as the transition progresses toward menopause. [2][10]
- Flow changes. Some cycles may be lighter due to a thinner lining from lower estrogen exposure. Others may be heavier because an anovulatory cycle allowed the lining to build up for longer than usual without the regulating effect of progesterone. [1][4]
- Spotting. Breakthrough spotting can occur with fluctuating estrogen levels, particularly around ovulation or with an unstable lining. [8]
Other factors that influence bleeding
Beyond hormones, other factors can influence bleeding patterns during this life stage. These include thyroid function (see our article on [thyroid problems vs. perimenopause]), fibroids or polyps (which become more common with age), certain medications including blood thinners, and stress, which can influence cycle regularity through its effects on the hypothalamic-pituitary-ovarian axis. This is exactly why bleeding changes shouldn’t be automatically and exclusively attributed to “just perimenopause” without considering the full picture.
What does the evidence show?
Research characterizing the menstrual transition, including the STRAW+10 staging criteria used widely in menopause research and clinical practice, confirms that cycle irregularity, specifically variability of seven days or more from a woman’s usual cycle length, is one of the earliest and most consistent markers of entering perimenopause. [10-11]
How common is heavy bleeding?
The numbers may surprise you. A large prospective US study (the SWAN Menstrual Calendar Substudy) found that prolonged menstrual bleeding of 10 or more days affected over 90% of women at least once during the menopausal transition, and nearly 80% experienced it three or more times. [3] About a third of women had three or more days of heavy bleeding on at least three occasions. [3-4]
In other words, heavy and prolonged bleeding during perimenopause is far more common than many women (and even some providers) realize. Popular culture suggests that periods simply become lighter and less frequent as menopause approaches. In reality, the opposite is often true before periods eventually stop. [4][9]
When bleeding patterns need evaluation
At the same time, clinical guidelines from ACOG are clear that certain bleeding patterns are not explained by perimenopause alone and warrant evaluation regardless of a woman’s age or life stage. [1][5] Research on abnormal uterine bleeding identifies structural causes (fibroids, polyps, hyperplasia, and less commonly endometrial cancer) as important causes to rule out, particularly with very heavy bleeding, bleeding between periods, or any bleeding after menopause has been reached (defined as 12 consecutive months without a period). [5][8][13]
The evidence supports a balanced view: cycle irregularity is expected in perimenopause, but specific bleeding patterns fall outside that expected range and should prompt evaluation. [1][8]
What women should know
- Not all irregularity is equal. A cycle that’s a week shorter or longer than usual is a very different situation from bleeding that soaks through a pad or tampon every hour, or spotting that occurs between otherwise normal periods. [1][14]
- Your history and age matter. The evaluation threshold and approach can differ depending on your age, personal and family history, and whether you’re early or late in the perimenopause transition. There’s no single bleeding pattern that applies to every woman the same way. [5][15]
- Any bleeding after menopause needs evaluation. Once you’ve gone 12 consecutive months without a period, any subsequent bleeding, even light spotting, should be evaluated. This isn’t meant to cause alarm. Most causes of postmenopausal bleeding are benign. However, it’s a pattern that should never be assumed to be “normal” without a clinical assessment. [4][16]
- Heavy bleeding has real physical consequences. What evaluation may involve
Evaluation depends on your specific symptoms and history, and doesn’t mean extensive testing for everyone. [5][8] A provider may review your medical history and menstrual pattern in detail, perform a pelvic exam when appropriate, confirm your cervical cancer screening status, and recommend labs such as a complete blood count and ferritin level. [5]
For women aged 45 and older with abnormal uterine bleeding, ACOG recommends endometrial biopsy as a first-line test to rule out endometrial hyperplasia or cancer. [5][15] Endometrial biopsy is also recommended for younger women with risk factors such as obesity, prolonged anovulation, or failed medical management. [5][15]
Transvaginal ultrasound is often the preferred imaging study and can help identify structural causes like fibroids, polyps, or endometrial thickening. [5][8] Many women will only need a thorough history and pattern review. Others, based on their specific risk factors or symptoms, may benefit from additional testing. [8]
Evidence-based treatment options
If bleeding is affecting your quality of life and structural or premalignant causes have been ruled out, several evidence-based options can help: [5][17-18]
- Levonorgestrel IUD (52 mg). A Cochrane overview found this to be the most effective first-line medical treatment for reducing heavy menstrual bleeding, performing as effectively as endometrial ablation. It also provides contraception, which remains relevant during perimenopause. [5][17]
- Tranexamic acid. An antifibrinolytic medication taken during heavy flow days (typically 1300 mg three times daily for 4–5 days per cycle) that can reduce menstrual blood loss by approximately 50%. It is a non-hormonal option for women who prefer to avoid or have contraindications to hormonal treatments. [5][18]
- Combined oral contraceptives. Can regulate cycles, reduce flow, and provide contraception, though they are contraindicated in women over 35 who smoke or those with certain cardiovascular risk factors. [1][18]
- Oral progestins. Cyclical or continuous progestins can help regulate anovulatory bleeding, though side effects including mood changes and irregular bleeding may limit tolerability for some women. [8][18]
- NSAIDs. Nonsteroidal anti-inflammatory drugs such as naproxen can modestly reduce menstrual blood loss and are often used alongside other treatments. [5][18]
- Iron supplementation. When heavy bleeding has led to iron depletion or iron deficiency anemia, iron replacement is appropriate. Current evidence supports 65 mg of elemental iron (ferrous sulfate 325 mg) administered every other day to maximize absorption while minimizing side effects. [5][7]
